- Generated On (Basic, Premium, and Ultimate)
- This line displays the date and time when the report was
generated.
- Search Terms (Basic, Premium, and Ultimate)
- This line displays the search terms that you entered into the
search form.
- Phone Risk Indicator and Risk Alerts (Premium and Ultimate)
- If the Phone Risk Indicator option is enabled by your administrator, then
the words Pass,
Fail, or Warn
indicate the risk level of the phone number. The Phone Risk Indicator uses
seven customer-configurable risk rules to return this result. A
Fail return hides the LexisNexis® One Time Password (OTP) icon () to ensure that you do not run the
OTP search on a
phone number that has triggered unfavorable activity returns.
If a
Warn or a
Fail Phone Risk Indicator appears, then
the customer-configurable rules that are used in this determination are
displayed.

Primary Phone |
- Phone Number (Basic, Premium, and Ultimate)
- This field provides the most relevant phone number that is associated with
the subject's information that you entered. If the Phone Risk Indicator
returns a Pass or a
Warn value, then you can click the OTP icon to access
the OTP window that
lets you send a unique one-time password in the form of an
SMS (short message service) / text
message, voicemail, or email to the subject to verify their identity. See
Phone Finder to One Time Password.
- Phone Type (Basic, Premium, and Ultimate)
- This field provides details about the phone type. Values for the
primary phone number include
Land-line, Possible
Wireless, Possible
VoIP, Prepaid, and
Other. The
Other indicator appears if LexisNexis® Risk Solutions
cannot determine the phone type.
- Status (Basic, Premium, and Ultimate)
- This field provides details about the current status of the phone
number. The values for the primary phone number are
Active or
Inactive. This information comes
from a real-time phone gateway source that determines whether the
phone number is live and billable.
- Activity (Ultimate)
- The value (Forwarded) indicates that
the phone number has been forwarded.
- Listing (Premium and Ultimate)
- This field provides details about the phone number listing type.
Values for the listing type include
Residential,
Business, and
Government.
- CallerID (Premium and Ultimate)
- This field displays the active caller ID name that appears for the
primary phone number. This information comes from a real-time
phone gateway source.
- Carrier (Basic, Premium, and Ultimate)
- This field displays the name, the city, and the state details for
the carrier that is associated with the primary phone number.
No Contract Carrier appears when
the carrier provides service does not require a contract
signature.
Premium and Ultimate reports include the
carrier's address.
- Ported (Basic, Premium, and Ultimate)
- This field displays the ported status. A ported phone is a phone number that
has been moved from one carrier to another. In Basic reports, if the phone
has not been ported, Not Ported is displayed.
If the phone has been ported, then Ported is
displayed.
Premium and Ultimate reports provide details for phone
numbers that have been ported at least once. Theses details include the
number of times that the phone has been ported, the service providers
that the phone was ported to or ported from, the first ported date, and
the last ported
date.
- Spoofed (Ultimate)
- This field displays the spoofing status. Spoofing is the practice of
disguising the origin of a phone number. If the phone has not been spoofed,
then the number 0 is displayed.
Ultimate reports include the number of
times that the phone has been spoofed, the service providers, the first
spoofed date, and the last ported
date.
- OTP (Premium and
Ultimate)
- This field displays the OTP status. If the
phone has never had a one-time password associated with it, then the number
0 is displayed. When the number is greater than 0, the one-time password
data includes whether the phone has been associated with the use of
OTP, the number of
times it has been associated with OTP, the first date
the one-time password was attempted, the last date the one-time password was
attempted, and if the one-time password was successful.

Name |
- Name (Basic, Premium, and Ultimate)
- This field displays the first name and last name of the subject who is
associated with the primary phone number. If the subject is deceased, then
Deceased appears below the subject's
name.
- LexID
- This field displays the subject's LexID
number when it is available. This ID is a unique identifier that
is obtained from most LexisNexis Risk Solutions
searches. See About LexID.
- Primary Name
- Primary Name appears below the name that is the
primary name that is associated with the phone number. If additional names
are associated with the phone number, then they appear below the primary
name.
- If the Ultimate option is selected, and if the identity has been verified
with the carrier, then Carrier Verified is
displayed.

Phone Dates |
- Last Seen (Basic, Premium, and Ultimate)
- This field returns where the subject was last seen in the LexisNexis Risk Solutions data
that is associated with the primary phone number.
- First Seen (Basic, Premium, and Ultimate)
- This field returns where the subject was first seen in the LexisNexis Risk Solutions data
that is associated with the primary phone number.
- Duration Seen (Basic, Premium, and Ultimate)
- This field displays a calculation, in months, of how long the
subject has been associated with the primary phone number.
- Seen (Basic, Premium, and Ultimate)
- This field displays a calculation, in months, taking the current
date and subtracting the last-seen date. This information details
how recently the subject has been associated with the primary
phone number based on the LexisNexis Risk Solutions
data.

Address |
- Address (Basic, Premium, and Ultimate)
- This field displays the subject's current address that is
associated with the phone number. If the latitude and longitude
are found, they are included.
- Status (Basic, Premium, and Ultimate)
- This field displays the status of the address that is associated with the
phone number. If the subject has a LexID number, and
LexisNexis Risk Solutions data
determines that the subject currently resides at the address, then
Current is displayed.
This field displays the classification of the property. If the subject has a LexID number, and LexisNexis Risk Solutions data contains the
property type, then the property type is displayed. Property types are
Residential or
Commercial.
